Why do you need my personal information?
Your personal information is used to assist with the diagnosis and treatment of your health, for accurate personal identification and communication. Specifically, the information is used for the following purposes:
- Disclosure to other health professionals involved in your medical care. This includes your general practitioner, other medical specialists, allied health professionals and medical investigation providers.
- For accounting and administrative purposes to comply with Medicare requirements.
- For quality assurance within my practice.
Your medical information will not be released to a third party not directly involved in your health care without your further written consent.
Appointment Cancellation or Non-Attendance Policy
Our doctors are committed to providing all of our patients with exceptional care. Our doctors strive to be available for your specific needs and also the needs of all our patients. When a patient does not attend a scheduled appointment, another patient loses an opportunity to be seen.
We respect that your time is valuable, and we appreciate that you understand ours is too. We understand that unplanned issues may arise and you may need to cancel an appointment on occasion. If this occurs, we ask you to contact us early to reschedule your appointment. Our appointments are in demand and early cancellation will allow another patient on the waitlist the opportunity of seeing the doctor.
If you need to make a cancellation, please contact us at least 48 hours or two business days before your appointment time by calling the rooms on (03) 9088 6525.
If your appointment is on a Friday at 11am, your appointment needs to be cancelled
before the previous Wednesday by 11am
Cancellation or changes to a scheduled appointment time within the above notice period will be charged a cancellation fee. Appointments that are not attended (including telehealth video and telephone consultations) will be charged a non-attendance fee.
- The late cancellation or non-attendance fee for an initial appointment is $150
- The late cancellation or non-attendance fee for a review appointment is $50
This fee has to be paid prior to booking any subsequent appointments. These fees are not rebatable through Medicare.
If there are extenuating circumstances which prevent you from cancelling your appointment with sufficient notice or resulting in your non-attendance, please feel free to discuss your concerns with one of our friendly reception staff.
